Student Learning Outcomes - Upon successful completion of this course students will be able to: - Demonstrate an understanding of the transitions in care model and the implications for nursing practice.
- Articulate an understanding of health care structure, financing and the implications for nursing practice and patient care
- Demonstrate an understanding of nursing case management models and practice through the presentation of a nursing case management model directed at population health or disease specific management.
