NURS 415 - Transitions of Care

Key concepts of the transitions of care framework and related content on health care history, financing, care settings, nursing case management and coordination of care.

Prerequisites: B.S. Nursing major and all of: NURS 320, NURS 321, NURS 322, NURS 326, NURS 327.
Possible Instructional Methods: On-ground, or Hybrid, or Online-Asynchronous.
Grading: A-F grading only.
Course Typically Offered: Fall & Spring


Student Learning Outcomes - Upon successful completion of this course students will be able to:
  1. Demonstrate an understanding of the transitions in care model and the implications for nursing practice.
  2. Articulate an understanding of health care structure, financing and the implications for nursing practice and patient care
  3. Demonstrate an understanding of nursing case management models and practice through the presentation of a nursing case management model directed at population health or disease specific management.
